Please click on Adult New Patient Forms or Pediatric New Patient Forms (age 2-14 years) and print out the pages. Allow at least 30 minutes to read and complete these 4 pages. These pages are essential to conplete and bring to your first visit appointment.

The Additional Adult Questionnaires are also very useful for your first visit. Please click on Additional Adult Questionnaires and print the 7 pages. These extra questionnaires will also take about 30 minutes to complete and will enable your naturopathic doctor to make a more thorough assessment of your health on the first visit. If you don't have time to complete the Additional Adult Questionnaires for your first visit, please fill them out later and bring them to your second visit.

Please allow yourself enough time to complete these pages in order to provide your naturopathic doctor with all the requested information to make your first visit appointment more effective and efficient.
